The "Central Ohio Case Management Network" is a non-profit organization dedicated to the support and development of the profession of case management through networking opportunities and educational forums.

COCMN began because of a strong commitment to provide much needed networking opportunities in the Central Ohio area. The group is patterned after NEOCMN, located in Northeast Ohio.

COCMN has continued to exist because of the strong commitment of its officers, board of directors, committee chairpersons and the involvement of its members and sponsors.

We are a non-profit multidisciplinary networking organization for case managers and health related professionals.

Case Management Definition

We define case management as a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual's health needs through communications and available resources to promote quality, cost effective outcomes.
